Abstract. Purpose:To report recovery of vision following surgery in a case of rhinogenic optic neuropathy. Case:A 30-year-old male presented with impaired vision and periocular pain in the right eye since 2 weeks before. Finding and Clinical Course:Corrected visual acuity of counting fingers right and 1.5 left. No pathological findings were present objectively, except constriction and central scotoma in the right visual field. Pulsed corticosteroid therapy was followed by improvement of right vision to 0.4 10 months later. He presented again with impaired vision and periocular pain in the right eye 5 years later. Visual acuity was 0.03 right and 1.2 left. Pulsed corticosteroid therapy induced partial improvement of vision 3 months later to be followed by further recurrence. Reexamination of former diagnostic imaging showed viscous material in the posterior ethmoidal cavity. Surgery was followed by improved visual acuity to 1.0 5 days later. He is doing well for 10 months until present. Conclusion:The present case was atypical and protracted optic neuropathy secondary to ethmoidal sinusitis. Surgery induced lasting recovery.
