Cushing症候群に合併した重篤な糖尿病性増殖性網膜症の1例

Diabetic proliferative retinopathy in a case of Cushing syndrome

　26年前Cushing症候群による左副腎摘出術をうけた45歳の女性の二次性糖尿病の1症例を報告した。症例は重篤な糖尿病性増殖性網膜症（以下網膜症）となり，両眼硝子体手術を受けた。網膜症の進行には，一次性糖尿病の遺伝因子を持ち，その上，副腎皮質結節性過形成の発症がtriggerとなり，血中濃度S-Cortisol，尿中17-OHCSの高い基礎値が，関与していることが推測された。本邦では3例目で，硝子体切除術および全周輪状締結術を施行したのは初例であった。Cushing症候群による二次性糖尿病性網膜症の発生と管理の重要性について述べた。

A 45-year-old female developed secondary Diabetes Mellitus after undergoing left adrenalectomy 26 years ago. She noted visual impairment bilaterally 3 weeks before, leading to the diagnosis of proliferative retinopathy with retinal detachment. We perfomed pars plana vitrectomy with success for both eyes. Diabeticproliferative retinopathy in this case seemed to involve three factors in its pathogenesis: presence of hereditary factor for Diabetes Mellitus, trigger of retinopathy due to adrenal nodular hyperplasia, and the high baseline value of serum cortisol and urinary excretion of 17-OHCS. We stress the neces-sity of regular ophthalmological follow-up in patients with Cushing syndrome even after surgery.
