隅角癒着解離術の手術成績

Results of goniosynechialysis for glaucoma

　閉塞隅角縁内障13例14眼に隅角癒着解離術（goniosynechialysis, GSL）を施行した。うち原発性閉塞隅角緑内障7眼，虹彩炎や外傷、手術などによる続発性緑内障7眼であった。対象眼の眼圧はfull medicationにて28〜43 mmHg，周辺虹彩前癒着は50〜100％の隅角を閉塞していた。Healon存在下，GSLを行ったが，症例によっては，core vitrectomy，水晶体嚢外摘出，周辺虹彩切除及びtrabeculotomyを同時に施行し，術後Laser gonioplastyを行った。2〜16ヵ月の術後経過観察期間を通じて，大多数の症例で点眼のみで眼圧のコントロールが可能となった。重篤な早期合併症はなかった。以上，GSLは安全で有効な手術と考えられた。

We treated 14 eyes with closed angle glaucoma by goniosynechialysis after the method by Camp-bell 1984 and Nagata 1985. After core vitrectomy and deepening the anterior chamber, we severed the peripheral anterior synechia under direct observa-tion. Laser gonioplasty was later applied to keep the angle open. The series included 7 eyes withprimary angle closure glaucoma and 7 eyes with angle closure due to iritis, trauma or surgery. The intraocular pressure was uncontrollable under full medication.

At the end of follow-up periods from 2 to 16 months, the intraocular pressure was well controlled in 10 eyes : 6 out of 7 eyes with primary and 4 out of 7 eyes with secondary angle closure.
