単純ヘルペス性脳炎に網膜炎を合併した新生児の1例

Retinitis developed in a neonate with herpes simplex encephalitis

　34週，2,2749で出生の新生児の単純ヘルペスⅡ型脳炎に合併した，両眼網膜炎の臨床像を記載した。脳炎に対し，アシクロビルが投与された。両眼網膜の赤道部から周辺部にかけて，全周に塊状黄色滲出斑を認めた。黄斑部には左眼に滲出斑を，右眼に黒色色素沈着を認めた。視神経は徐々に萎縮した。滲出物は徐々に吸収し，跡は黒色色素沈着を伴った瘢痕を呈した。アシクロビルが有効であったと考えられる。新生児単純ヘルペス脳炎は網膜炎が合併しうることを強調した。

A male infant was diagnosed as herpes simplex type Ⅱ encephalitis 22 days after birth. He was born at 34 weeks of gestation and weighed 2,274 g. The clinical findings improved after intravenous acyclovir. Funduscopy 52 days after birth revealed massive yellowish exudates along the entire cir-cumference peripheral to the equator in both eyes. The macula was affected by exudates in the left eye and by pigmentations in the right. Optic atro-phy gradaully developed in both eyes. The exudates in the peripheral fundus became slowly absorbed leaving retinal scars. Acyclovir was effective for herpes simplex virus encephalitis and retinitis.
