汎網膜光凝固後増悪した黄斑浮腫と滲出斑に対する追加光凝固

Additional photocoagulation for progressive subfoveal exudate and macular edema after panretinal photocoagulation

　糖尿病性網膜症にたいして汎網膜光凝固（PRP）を施行した後，黄斑部のびまん性浮腫が増強し中心窩に滲出性白斑が沈着した7例に対して，黄斑部に乳頭黄斑線維束を除く馬蹄形のアルゴンレーザー追加凝固を施行した．

　全例で滲出斑は軽快，消失し検眼鏡所見は改善したが，6ヵ月後の視力は，改善1例，不変5例，悪化1例であった。自覚所見は5例で改善し，悪化したものはなかった。滲出斑をきたした症例は高血圧，腎症などを合併した例が多く，他眼にも黄斑部病変が強くみられた。

　このような黄斑部追加凝固の適用により検眼鏡所見，自覚症状は改善するものの，視力回復には有効とはいえなかった。PRP後に黄斑浮腫が増悪した症例には早期に追加凝固の適応を考えるべきであり，さらにそのような症例を事前に判別し，視力低下をきたさないPRPの方法が考案されるべきであると考えられた。

Seven eyes in 7 diabetic patients developed sub-foveal hard exudate and diffuse macular edema following panretinal photocoagulation. We treated these eyes with additional argon laser photocoagulation placed in a horseshoe pattern in the parafoveal area excepting the papillomacular nerve fiber bundle.

Subfoveal exudates either decreased or disappeared in all the 7 eyes. The visual acuity improved in 1, remained stationary in 5 and deter-iorated in 1 eye.

Four patients in the series also suffered from hypertension or diabetic nephropathy. Maculopa-thy was present in the fellow eye in the majority of the cases.

The present approach with additional photocoagulation is thus of limited value as it can induce improvement in fundus findings but not in visual acuity.
