Insulin-dependent diabetes mellitus was detected in a 20-year-old female 3 years before. The glucose control has been poor, with occasional ketoacidosis or hypoglycemia. She has been under more rigoroustherapeutic regimen for the past 5 months.

Fluorescein fundus angiography showed, bilater-ally, capillary dilatation and multiple areas of hyperpermeability in retinal areas where radially peripapillary capillareis are distributed. As the visual acuity in the left eye decreased to 20/30 2 months later, we treated the eye with panretinal photocoagulation. Three months, later, retinal edema subsided spontaneously. The visual acuity in the left eye improved to 20/20. It appeared that such transient, though pronounced, capillary hyperper-meability of the retina may have been caused by variable changes in the plasma glucose level.
