眼筋手術の量定に関する研究—予報 眼位の測定について

Quantification in extraocular muscle surgery. : Measurement of eye position (preliminary report)

　眼筋手術の量定を目的として，術前，術後の眼位を正確にかつ定量的に測定するために，写真およびビデオカメラによる撮影計測を行った．写真撮影は望遠，接写装置にプリズムを併用して角膜写真像の拡大をはかり，この角膜反射像より斜視角を測定した．

　カメラの移動撮影により，斜視角またはむき眼位よりの角膜反射像が明確になり，正確な計測ができるようになり，特に上下偏位のある際に有利である．

　下斜筋過動の程度を表わすために，患眼の30°内転時の上方偏位角を用いることとした．これにより，mm単位で表わされる下斜筋後転術との比較において下斜筋の手術量定を数量的な関係として検討することが可能となる．

　ビデナヵメラを使用して，眼球運動，眼位の状態を記録し，手術の量定法に役立てることとした．

Exact measurement of eye position is a prere-quisite for quantification in the surgical correction of extraocular muscles. We determined the angle of strabismus by recording the corneal reflex photo-graphed with a camera with attached prism on the telescopic lens.

Photographs taken from the direction of deviation were valuable in determining the angle of vertical deviation of the squinting eye. In eyes with overac-tion of inferior oblique, measurement of the amount of hyperdeviation during inner gaze by 30° was of value in pre- and postsurgical evaluations of recession of the inferior oblique muscles.

A video camera can be attached to our system to record eye movement and is also expected to be of value in evaluating the effect of surgical treat-ment.
