We observed unexpected allergic external eye reactions to instilled mydriatics in 15 patients during a 1-year period. We attempted to identify the causative component in the mydriatic solutions using patch test in these patients.

There were 6 patients who allergic to topical Mydrin PR and not to NeosynesinR. They mainly manifested hyperemia of the conjunctive. In these patients, patch test was positive to benzalkonium chloride which was contained in the ophthalmic solution as presertive.Benzalkonium chloride was thought to be the allergen for them.

There were 9 patients who were allergic both to Mydrin PR and NeosynesinR. They showed mainly edema and redness in the eyelids. In these patients, patch test was positive to phenylephrine hydrochloride which was thought to be the aller-gen for them.
