内頸動脈血行不全による虚血性網膜症の2例

Two cases of ischemic retinopathy due to stenosis of internal carotid artery

　内頸動脈狭窄あるいは閉塞に起因する虚血性網膜症2症例を報告した。

（1）眼底所見は，症例1は網膜耳側動脈閉塞症所見を，症例2は網膜中心動脈閉塞症を伴う前部虚血性視神経症の所見を呈した。螢光眼底造影検査で2例共に脈絡膜螢光充盈遅延・網膜循環時間の遅延を認めたが，特に症例2ではそれが著しかった。

（2）頸動脈血管造影で症例1では，左内頸動脈の起始部に著明な狭窄と眼動脈の閉塞所見を認め，症例2では右内頸動脈起始部の完全閉塞と外頸動脈から眼動脈へ吻合する側副血行路の形成が認められて，2例ともに内頸動脈の狭窄あるいは閉塞による虚血性網膜症であることが診断された。

（3）治療はウロキナーゼによる血栓溶解療法を施行したが無効で，特に症例2では虹彩ルベオージスが進行して血管新生緑内障を続発し，眼圧のコントロールができず，強い眼痛のために眼球摘出術を施行した。

We observed ischemic retinopathy due to stenosis of internal carotid artery in a 49-year-old male and 54-year-old female. In the first case, amaurosis fugax in the left eye was followed by contralateral hemiplegia and aphasia. Occlusion of superior temporal retinal artery was present. Carotid angi-ography revealed a marked stenosis of left internal carotid at the site of bifurcation as well as occlusion of the left ophthalmic artery. The second case had suffered from right cerebral infarction 9 months before. Sudden visual impairment with ocular pain had set in 2 days before.
