下甲介折骨術による先天性鼻涙管閉塞の治療

Inferior turbinotomy in congenital nasolacrimal duct obstructions

　ブジー法が無効の先天性鼻涙管閉塞10例に下甲介を正中側にむかって骨折させる方法（Jones　＆　Wobig）を追試した。内6例は下甲介嵌入，4例は閉塞部の線維増殖と考えられたが，前者の4例と後者の2例に卓効を得た。24歳の成人例にも奏効したことは特筆に値する。本法は先天性の頑固な鼻涙管閉塞に対して，シリコン留置術あるいは涙嚢鼻腔吻合術に代わる，簡単かつ有効な手段である。

Ten patients with congenital and persistent obstruction of the nasolacrimal duct in spite of simple probings were surgically treated by infrac-turing the inferior turbinate described by Jones and Wobig. Functional cure resulted in 4 of 6 patients with impacted turbinate and 2 of 4 cases with fibrosis of the obstruction could be cured. It is worthy of note that a 24-year-old male responded favorably to this treatment. The results suggest that the procedure is a useful alternative to more com-plicated techniques, such as silicone intubation or dacryocystorhinostomy.
