活動期未熟児網膜症の網膜剥離に対する硝子体手術

Vitrectomy in infants with retinal detachment caused by retinopathy of prematurity during the acute phase

　激症型未熟児網膜症の活動期病変において網膜剥離を来たすのは牽引性の要素が強い。

　その機序は超音波検査により赤道部付近から硝子体腔内に向かって襖型の像がのびるのが認められ，次第に水晶体後面でいわゆるcyclitic membrane様の像を形成して網膜が剥離してくる。

　したがって手術は光凝固や冷凍凝固を行っても網膜剥離が生じてくる場合に，牽引を除去するためにこのcyclitic membrancの形成をさまたげるように，中心部および前部硝子体を切除すべきで，そのためには水晶体切除を併用した方がよい場合もある。

We performed pars plana vitrectomy on 9 eyes of 6 premature infants for retinal detachment during the active phase of retinopathy of prematurity (ROP). Successful reattachment of the retina result-ed in 5 eyes, of which detachment recurred 2 years later in one eye.

Funduscopic and ultrasonographic findings sug-gested that the retinal detachment in ROP was caused by vitreal traction even in the active phase.

In the initial stage of severe active ROP, a wedge-shaped figure appeared in the vitreous at the equator but no retinal detachmant ultrasono-graphically.
