A 22-year-old female manifested congenitalabsence of the inferior rectus muscle in both eyes as a rare anomaly. Her past history indicated the ab-normal ocular motility to be present since birth and chin depression since childhood. Her left eye deviated upwards on left gaze and the right eye deviated upwards on right gaze. Each eye could not be depressed below the horizontal midline while in abduction.

Computed axial tomography indicated an absence or hypoplasia of both inferior rectus muscles. Neither the inferior rectus muscle nor its insertion to the sclera was found during surgery in each eye.
