眼瞼蜂窩織炎で発見された再生不良性貧血と思われる1例

A case of aplastic anemia with eyelid phlegmon

　眼瞼蜂窩織炎で発見された二次性再生不良性貧血の1例を報告した。症例は60歳の男性で感冒のため抗生物質，鎮痛解熱剤，抗ヒスタミン剤などを内服していたが，齲歯を抜歯後発熱が続き，左眼に強い眼瞼蜂窩織炎が発症し，全身検査で汎血球減少症が認められた。大量の抗生物質，輸血，免疫グロブリン製剤の投与を行ったが蜂窩織炎は頬部，頸部，前胸部，後頭部に拡大し，初診から17日目に肺炎，心不全，播種性血管内凝固症候群（DIC）を合併し死亡した。骨髄穿刺，剖検はできなかったが発症の約1カ月前の血液検査で異常はなかったこと，末期に白血球数，血小板数の回復傾向がみられたことなどから薬物による二次性再生不良性貧血と考えられた。

A 60-year-old male developed swelling of the left eyelids. He had been under medical treatment for common cold for the past 4 weeks. He had one of his teeth removed for caries one week ago. He was diagnosed as eyelid phlegmon and was admitt-ed to our hospital. Laboratory tests showed severe pancytopenia : WBC 600/mm³ with no granulocyt-es, RBC 306 X 10⁴/mm³ and platelets 3,000/mm³. It was presumed that secondary aplastic anemia was caused by drugs for common cold. In spite of energetic treatment with antibiotics and blood transfusion, the phlegmon extended to invade thecheek, neck head and the thorax.
