真菌性眼内炎の治療

Clinical management of fungal endophthalmitis

　我々は今回，抗生剤やステロイドの投与を受けた50歳男性に見られた真菌性眼内炎を経験し，副作用に対処しながらもAmphotcricin Bを投与して治癒に導きえた。

　真菌性眼内炎は，全身性真菌症の一つの表われであるばかりでなく，それ自体，適当な処置がなされねば失明するという点で重要な眼疾患の一つである。我々の症例にも見られるごとく抗生剤やステロイドの投与，その他手術，全身状態の悪いものなどが素因となる。初期には確定的な臨床症状に乏しく，また培養同定が困難であることなど，本疾患の早期診断には困難な点が多い。

　Amphotericin Bは副作用が強く，眼内移行が悪いが，他に有力な薬剤が無い以上，第一選択となる薬剤である。副作用としては，発熱，悪寒に始まり種々にわたるが，多くは対症的に克服可能である。我々の症例では，30mg／日の維持量で，約1gの使用で治療効果をみた。

A 50-year-old male developed bilateral fungal endophthalmitis after systemic treatment with antibiotics and corticosteroid for trauma-induced bowel problems. Fundus endophthalmitis was ten-tatively diagnosed on clinical features and on posi-tive culture of Candida albicans from urine. Sys-temic treatment with Amphotericin B was started, initially with a very small dosage to be increased to 30mg every other day and to total lg over two months. Due precautions were taken to minimize side effects : analgesics and corticosteroids for fever and heparin for phlebitis.
