アトピー性皮膚炎の病型からみた網膜剥離の臨床像

Clinical features of retinal detachment according to type of atopic dermatitis

　網膜剥離を発症したアトピー性皮膚炎で，皮膚科的所見が明らかな19例につき，皮膚病型により，網膜剥離の臨床像に違いがあるかを検討した，皮膚病型を大きく乾燥型と湿潤型に分類すると，1987〜1992年は80％が乾燥型であったが，1993年以降は，湿潤型が72％を占めた。乾燥型では白内障と前房混濁が有意に多かった。湿潤型では，硝子体基底部裂孔と不定形裂孔の他に，網膜格子状変性を伴わない弁状裂孔や円孔が原因裂孔としてみられた。皮膚病型の違いにより，網膜剥離の臨床像に違いがあった。

We examined 19 cases of retinal detachment associated with atopic dermatitis. We classified the atopic dermatitis into three types, dry type, wet type, and flash type. Up to 1992, 80% of the atopic dermatitiswas of the dry type, but after 1993, 72% was of the wet type. The patients in all dry type cases also had cataracts, and many of them had floaters in the anterior chamber. The patients in wet type cases had a round hole or flap tear without lattice degeneration.

In conclusion, we suggest that there are several different clinical features of retinal detachment associated with atopic dermatitis, according to the type of dermatitis.
