白内障手術と角膜内皮細胞の術後変化—広視野スペキュラーマイクロスコープと画像処理装置を用いて

Corneal endothelial changes after cataract extraction studied by a wide-field specular microscope and an image analyzer

　白内障症例の嚢内摘出術・計画的嚢外摘出術と人工レンズ移植例の角膜内皮損傷を，広視野スペキュラーマイクロスコープを用いて術後1ヵ月目の角膜内皮細胞減少率として比較検討した。角膜中央部での平均細胞数減少率は嚢内法12.2±1.6％（40眼），嚢外法7.8±3.4％（12眼），人工レンズ移植例22.6±6.0％（6眼）（平均±標準誤差）となり，嚢外摘出法と嚢内摘出法は同様の値となった。パノラマ角膜内皮像からは，角膜中央より1〜2mm上方に細胞損傷の強い症例が示され，角膜全体の手術侵襲を知るためには角膜中央部の観察だけでなく，広範囲の角膜内皮観察と部位別損傷度の比較の重要性が明らかとなった。

Corneal endothelial damage after cataract ex-traction was studied using a wide-field specular microscope and a computerized image analyzer. Percentage endothelial cell loss at the centralcornea after intracapsular, planned extracapsular cataract extraction and intraocular lens implant surgery was 12.2±1.6 (40 eyes), 7.8±3.4 (12 eyes) and 22.6±6.0 (6 eyes), respectively, at one month postoperative interval (mean ± S.E.M), suggesting the same low endothelial damage caused by intra-and extracapsular cataract extraction.
