先天性眼瞼下垂の手術と視機能

Visual functions after surgery for congenital blepharontosis

　1976年より5年間の先天性眼瞼下垂手術180例中，112例についてfbllow upし，視機能検査を行って，次の結果を得た。

　今回の手術方法はBerkeの変法によったもので，手術年齢は1,2歳が44症例と多く，比較的早期に手術を施行している。手術の定量は挙筋作用4mm前後の場合，17〜18mmの切除量で行い，全体として64％にほぼよい矯正が得られた。

　下垂眼に高度の弱視は少ないが，0.3以下の症例が12例（10.7％）あり，斜視は23例（斜視8例，斜位15例）にみらねた。手術年齢と視力障害の関係は明らかでないが，下垂眼に乱視の多いことは特徴的で，今回の結果から強度の片限性下垂の場合は早期に手術するのがよい印象を得た。

We evaluated the postsurgical outcome in 112 out of 180 treated cases of congenital blepharoptosis in the 5-year-period up to 1980. We paid particular attention to the state of visual function including amblyopia, refractive errors, ocular deviation and binocular function. All the cases had been operated by modified Berke's procedure. For instance, a case in whom the levator function was limited in 4 mm was treated by meeting 18 mm of the levator muscle. Following results were obtained.
