糖尿病性網膜症の治療に及ぼすDextran Sulfateの影響

The effects of dextran sulfate on the treatment of diabetic retinopathy

　内科医の治療下にある糖尿病症例で網膜症を有するものを無作為に64例を対象とし，MDS 1,350 mg／dayを約1年間内服せしめ，その前後の血清脂質，凝固線溶系，および眼底所見を比較検討し次の結論を得た。

（1）血清脂質、血液凝固線溶系の改善が認められ，TG （P＜0.01），　Fbg （P＜0.01）の有意減少，FFA （p＜0.01），　ELA （p＜0.01）の有意増加が認められた。

（2） background retinopathy 115眼のうち眼底所見に改善が認められたもの21眼（10％），不変64眼（58％），悪化39眼（32％）であった。

（3）眼底所見が改善あるいは不変と認められたものは，大半が糖のコントロールが良好にしてかつ，肥満，遺伝素因，蛋白尿，高血圧などのrisk factorが低く，これに反し，proliferative retinopathyに増悪進行した症例の大部分は，遺伝素因を有し，蛋白尿強陽性という特徴を示した。

We treated 64 patients (128 eyes) with diabeticretinopathy by oral adminsitration of dextran sul-fate at the daily dosis of 1,350 mg for one year. The blood sugar level was controlled by internists. We examined the changes in fundus findings as well as total cholesterol, triglyceride, free fatty acid, fib-rinogen, prothrombin time, maximum amplitude in thromboelastogram and euglobin lysis area.

The treatment resulted in a significant decrease in triglyceride and fibrinogen (p<0.01), and in an increase in free fatty acid and euglobulin lysis area (p<0.01).
