開瞼失行症の5症例

Five cases of apraxia of lid-opening

　Apraxia of lid-opening開瞼失行症の5症例を報告した。症例1は76歳女性，半年前に開瞼因難が発症する。開口運動あるいは頸部後屈の動作により開瞼可能となる。症例2は左片麻痺の58歳女性。症例3は65歳女性で，CT scanにてdilfuse cortical atrophyを認めた。症例4は48歳男性，精神状態の変化，あるいはストレスの多寡により症状に相当の変動が認められるのが特徴である。症例5はアルコール中毒の54歳男性。本症は症候性眼瞼痙攣より開瞼失行症へ移行したと考えられる症例である。

　開瞼失行症の鑑別診断問題点につき若干述べるとともに，眼科領域におけるその重要性につき強調しさらに，これまで開眼失行症という不適切な日本語が用いられてきたことを指摘した。

Apraxia of lid opening is defined as difficulty in the induction or maintenance of lid opening follow-ing supranuclear lesions. We observed 5 cases with this condition. All the cases were advanced in age ranging from 48 to 76 years. Three were females and two were males. The etiological lesions were variable. Hemiplegia with systemic hypertension was present in one and diffuse cortical atrophy in another. In the third case, there was excessive myopia in one eye and total retinal detachment in the fellow eye. The symptom fluctuated with the emotional mood of the patient.
