Penetrating keratoplasty was performed in a 58-year-old male with heavily vascularized and opa-que cornea. A 7 mm trephining and suture with 10-0 nylon were employed. The donor cornea was of a 80-year-old female removed 2 hours postmor-tem and preserved for 24 hours in preservation media. After an initial favorable response, diffuse corneal edema and keratic precipitates appeared 6 weeks after surgery. Graft rejection was diagnos-ed. A second penetrating keratoplasty was neces-sitated 9 weeks after the initial surgery. The excised specimen was subjected to histological and ultra-structural studies.
