人眼における角膜拒絶反応—走査型ならびに透過型電顕による観察

Corneal graft rejection in a human eye.: A scanning and transmission electron microscopic study

　58歳の男性の血管侵入が高度に認められた角膜白斑に対し全層角膜移植術を施行した。術後約6週間経過して拒絶反応による移植片の浮腫を生じた。ステロイド治療に反応せず，透明治癒が望めなかったため，再度角膜移植術を施行した。その際に見られた角膜紅織の走査ならびに透過電顕的観察により，この移植片の浮腫は，母角膜より手術創を経由し移植片内皮に達したリンパ球により生じた内皮障害の結果起ったものと推定された。また木症例に認められた所見は今までに報告されている動物実験結果とよく一致していた。

Penetrating keratoplasty was performed in a 58-year-old male with heavily vascularized and opa-que cornea. A 7 mm trephining and suture with 10-0 nylon were employed. The donor cornea was of a 80-year-old female removed 2 hours postmor-tem and preserved for 24 hours in preservation media. After an initial favorable response, diffuse corneal edema and keratic precipitates appeared 6 weeks after surgery. Graft rejection was diagnos-ed. A second penetrating keratoplasty was neces-sitated 9 weeks after the initial surgery. The excised specimen was subjected to histological and ultra-structural studies.
