慢性関節リウマチに合併した網膜中心動脈閉塞性病変の1例

A case of occlusive retinal angiopathy with chronic rheumatoid arthritis

緒　言

　慢性関節リウマチの眼合併症としては，前眼部病変の報告は多数認めるが，眼底病変合併の記載は極めて少ない。今回著者は両眼に急激なる視力障害を自覚した患者の眼底に網膜中心動脈の閉塞性病変をみ，螢光眼底撮影により，それが血管炎によるものであると考え，最近の慢性関節リウマチに関する病理学的な報告例の検討から，原病に伴うものであろうと推論した。

A 52-year-old housewife suffering from rheu-matoid arthritis for the past 6 years complained of blurred vision in both eyes after cessation ofsystemic steroid therapy. There were no abnor-mal findings in the anterior segment of the eyes. Ophthalmoscopical examination revealed slight disc pallor, prepapillar neovascular net, exudates in macular area, superficial retinal haemorrhages, arterial narrowing and obstruc-tion, venous dilatation and tortousity. These retinal changes were interpreted as due to angitis in retinal vessels peculiar to rheumatoid arthritis on following reasons:
