広大眼科教室で最近5年間に観察した後部硝子体剥離について

Statistical Study on Posterior Vitreous Detachment in Hiroshima University Hospital

I．緒言

　後部硝子体剥離の診断は，細隙灯で，網膜から離隔した硝子体後境界層を検出する事で確定される。しかしこの病変が後極部に及んだ際には，高い率で特異な乳頭前硝子体混濁斑を伴うので，少くともこの混濁斑を認めた場合には，Rieger等も指摘したように，直ちに後部硝子体剥離と判断して差支えないものと考えられる。

　従つて，乳頭前硝子体混濁斑を忠実に追求するならば，徹照，倒像，直像検査などの，我々が日常実施する手段だけで，既に本症を診定し得るのである。但し，この手順では，上方硝子体剥離等の，乳頭前混濁斑を有しない後部硝子体剥離は捕捉されないから，実在よりも尚少い頻度が検出される理である。

During the period of 5 years from 1953 to 1957, 236 cases (280 eyes) with posterior vi-treous detachment were observed in our eye clinic.

Incidence of the posterior vitreous detachment, diagnosed by means of ophthalmoscopic confirmation of the prepapillary vitreous spots, was relatively high showing 1.3% in average in all patients who consulted our clinic.

As to prepapillary vitreous opacities, spots of ring-haped and incomplete ring-shaped opacities were found in many cases. Biostatistically, however, incidences of other types of opacities were more frequently observed than those of the above spots.
