We prospectively studied 136 eyes of 118 patients who visited us for floaters as chief complaint. We paid particular attention to posterior vitreous detachment (PVD) as the underlying cause of floaters. The vitreous was examined using a preset convex lens and a three-mirror lens. PVD was present in 102 eyes (75%) . It was present in 100 of 108 eyes (93%) in patients aged 50 years or over, and in 2 of 28 eyes (7%) in those aged 49 years or less. The difference was significant (p < 0.01) . In 96 eyes in patients who noted floaters in the temporal visual field, 86 eyes (90%) showed PVD with prepapillary opacity in the detached posterior hyaloid membrane shaped as a ring, a ball or a line. In another 38 eyes in patients who noted floaters in the central visual field, 14 eyes (37%) showed PVD. The difference was significant (p <0.01). The findings show that prepapillary ring secondary to PVD is a major cause of floaters in patients who are 50 years or over and who note one or two dense floaters in the temporal visual field.
