内眥贅皮（Epicanthus）の手術について

Consideration of the surgical correction of epicanthus

1．緒言

　眼瞼皮膚の半月状皺襞がその凸面を鼻梁に向けて内眥部を被う様に縦走している変形に就て始めてSchon氏（1828）が記載し，von Ammon氏（1831）が"Epicanthus"と呼称したが，この内眥贅皮は多くは胎生3〜6箇月の頃に起る先天性異常で，凡ての人種に認められるものである。而してその発生機序は，眼窩内の中胚葉組織で眼筋に分化すべきものが発育変形したものと，内眥部を作る諸種組織の変形せるものとある様である。恒に両側性に起るが，その程度に差違を来すことあり，又皮膚皺襞の上眼瞼に止まるもの，下眼瞼へ及ぶもの，他種の先天畸形を伴うものなど種々ある。v．　Ammon氏（1860），Duke-Elder氏（1951）^1）等に依れば，i）贅皮が眉毛部に発し，涙嚢或いは鼻孔に到るもの（Epicanthus supracili-aris），ii）上眼瞼より発して眼窩下縁に及ぶもの（Ep．　palpebralis），iii）眼瞼瞼板に発し，内眥部に於てその皮膚皺襞が閉鎖するもの（Ep．tar-salis），iv）皺襞が下眼瞼より発して内眥部に及び，得てして眼瞼下垂，瞼裂狭縮症等の先天異常をも伴うもの（Ep．inversus）に分けることが出来るという。

　この外，後天性にも外傷により瘢痕を生じ眼瞼皮膚が牽引されて起るもの（Ep．　traumaticus）がある。

The author concluded as follows:

1) Operationsmethod for epicanthus should be selected depending upon the grade of severity and the variety which was classified according to the developmental base and the existing spot of this disease.

2) Operation for epicanthus has many variants, and is divided into two main classes, that is to say, resection of the skinflaps and displacement of the flaps at large. As auxiliary procedure to this operation the canthoplasty (by von Ammon) and the tucking of canthal ligament (by Johnson) would be used jointly.
