蚕蝕性角膜潰瘍の一治療法

Treatment of Mooren's Ulcer

緒言

　蚕蝕性角膜潰瘍は，比較的稀な疾患ではあるが，進行性のもので，多くは角膜周辺から中央にまで潰瘍が拡がる予後不良な疾患である。今までにも，いろいろな治療法が行なわれてきたが，本疾患の原因，作用機序が不明なため，治験例も散見されるにもかかわらず，適切な治療法というものは，まだ確立されていない現状である。われわれは今までには潰瘍部の結膜被覆や表層角膜移植，ステロイド療法等を行なつてきたが，いずれも長期間における効果をなずに終つている。

　今回，本疾患2症例において角膜潰瘍部の掻爬，周囲結膜の切除，自家血清，cysteineまたはステロイド点眼等を併用した治療を行ない，良好な結果を得た。

Two cases of Mooren's ulcer were treated by excision of limbal conjunctiva adjacent to the ulcer, topical serum, cysteine and steroid administration.

First case was a 54-year-old man who was treated first with conjunctival flap three times in the right eye, but in vain. Conjunctival excision in the vicinity of the ulcer stopped the progress of the ulcer until the exposed sclera was covered with proliferated conjuncti-va. Conjunctival excision was performed two times thereafter. There have been no recurre-nces for 6 months.
