Ocular Complications of Zygomatico-Maxillary Fracture

緒言

　頬骨は眼窩を構成している骨のうち，もつとも外力を受け易く，その骨折は顔面中央1／3の骨折のうちもつとも頻度が高い。頬部の骨折はいわゆるblow-out fractureとことなり，必らずしも眼症状を伴わないため，眼科医の診察をうける機会も少なく，眼合併症は比較的少ないと報告されていた。

　外傷により頬骨が，変位，骨折する場合には，周囲の前頭骨，側頭骨，蝶形骨あるいは上顎骨の骨折を伴う。このうち眼窩底を構成している上顎骨および頬骨の離解，骨折は，眼機能障害に関係が深い、，題名を頬骨（上顎骨）骨折としたのは，頬骨骨折に伴つて生ずる上顎骨骨折を強調する意味，およびLe Fort　Ⅱ，Ⅲ型を合併した症例を含めた頬部の骨折という意味である。

Ocular complications of forty-three cases of zygomaticomaxillary fracture (malar fracture) were discussed.

While very few intraocular complications were reported previously, ocular complications including, rupture bulbi, optic atrophy, corneal opacity and choroidal rupture were noted in 46% of this series. Ocular motility was involved in 63% and the involvement of palpebral and la-crimal drainage system was seen in 58% of cases.

The incidence of ocular involvements was significantly high in this series even though the patients were mostly referred from other physicians.
