Treatment and Course of a Typical Case of Uveal Effusion

緒言

　1963年SchepensおよびBrockhurst^1）は，Uveal effusionなる標題のもとに一種のブドウ膜疾患17例についての報告を行ない，その臨床像における特殊性から新たなclinical entityであると主張した。本報で述べる典型例とは，彼らが挙げた臨床所見の特徴にほとんど一致する一症例である。本症は長い年月にわたつて再発をくり返しながら徐々に悪化する傾向のあることが指摘されているが，著者らの例は最終的とも思われる高度の発症を起こすと同時に来院している。その後，2年間にわたる眼，全身症状と，治療に対する反応の経過を観察した結果，本症は原田病や周辺性ブドウ膜炎，あるいは漿液性網脈絡膜症などとは異なる，やはり非常に特殊な，そしてかなりまれな眼病であるとの印象を深くした。

　施行した手術的療法が確実に奏効したという所見は得られなかつたが，現在かなりの視機能障害を残しながらも，一応治癒とみなしうる状態に達したので，Schepensら^1）が触れなかつた螢光眼底造影の新たな所見と併せて報告しておきたいと思う。

A typical case of uveal effusion is presented. A 41-year-old man was referred to our depart-ment for treatment of retinal detachment of the right eye in November, 1972. Vision was reduced to 0.1(n.c.). The lower retina was de-tached in the right eye and progressed to total detachment in a few days. A silicon tube was inserted around the equator with tapping of subretinal fluid. The retina was reattached about a half year after the operation. In Decem-ber of 1973, similar changes occurred in the left eye in which the lower retina was detach-ed.
