視神経乳頭のMelanocytoma

Two Cases of Melanocytoma of the Optic Disc

緒言

　眼科領域における各種の腫瘍のうち，視神経原発の腫瘍は非常にまれなものであり，その報告も少ない。このうち，視神経乳頭にみられる色素性腫瘍においては，その良性，悪性の鑑別に著しい困難をきたすことが多い。

　われわれは最近，2例の乳頭上色素性腫瘍を有する症例を経験し，第1例目は臨床経過が進行性で諸種の検査結果より悪性黒色腫が疑われたため，眼球摘出を行ない，第2例目はmelanocy—tomaとして保存的に経過を観察中であるが，これらの症例について若干の検討を加えたので報告する。

Two cases of melanocytoma of the optic disc are presented.

The first case, a 36-year-old male, complained of blurred vision and visual field defects in the left eye. Funduscopy revealed a large pigmented tumor measuring more than 10 disc diameters. The eye was enucleated under the clinical dia-gnosis of malignant melanoma of the optic ne-rve or juxtapapillary malignant melanoma of the choroid. Histological studies showed plump and polyhedral cells with abundant cytoplasm and small nuclei. These characteristics led to the diagnosis of benign melanocytoma of the optic disc.
