頭頸部外傷症候群の眼障害と治療について

Ocular Involvement and Treatment in the Syndrome of Head-Neck Injury

緒言

　むち打ち症は衆知のように受傷機転を示すもので，傷病名ではない。一般にむち打ち症にさいしては頭部の打撲を伴うことが多い。たとえ頭部の打撲がない場合でも，頭蓋内と関係する種々の症状を生ずる場合があり，また，頭部外傷のさいにも「むち打ち症」同様の機転を生ずることも多い。したがつて頭部外傷とむち打ち症の眼障害は厳密な区別ができないため，これらを総称して「頭頸部外傷症候群」と呼んでいる。

　このように頭頸部損傷にさいして生ずる症状には，眼科のみならず，整形外科，脳神経外科，耳鼻科および精神科の各領域におよぶ多彩な症状を示す。場合が多いようである。

A report is presented on 109 cases of blunt trauma to the head or the neck region including the whiplash injury.

While asthenopia was the most frequent com-plaint following such injuries, the subjective signs could be classified as accommodation dis-turbance (86%), impaired convergence (66%), impaired accommdative pupil reflex, altered re-fraction, orthostatic fluctuation of blood press-ure in the central retinal artery, abnormal eye movement or eye position and abnormal visual field.

The ocular symptoms tended to persist for over months with little tendency to spontaneous recovery.
