眼前15mmに装用の眼鏡と不等像視

緒言

　眼鏡矯正にさいしレンズと角膜との距離を15mmにすれば，眼の屈折度のいかんを問わず網膜像の大きさが一定なりとの記載が，わが国の眼科教科書や成書に稀ならずみられる。ところでaniseikoniaの成立には網膜像のみならず網膜から脳に至る過程も関与すると説明されているが（保坂^1）），網膜以降の過程を現段階では数量的に律することができず，かりにその存在を無視すれば，前記の記載は眼鏡装用距離を一定にするだけで不等像視を消滅し得るとの意味になり得る。しかし実際の不等像視の矯正には別な光学的手段が駆使されるわけだし，凹レンズ眼鏡で矯正された強度近視眼が見る像が小さいのは歴然たる事実であつて，冒頭に引用の記載との食い違いがある。

　なお上記の15mmは眼の前側焦点の位置を想定したもので，成書によつては「眼の前側焦点にレンズを置けば」としたものもあるが，それも前側焦点が眼前一定距離にあることを前提としたもので，眼鏡矯正の実態と異なることに相違はない。なお眼生理学書The Eye，第4巻にBennetand Francis^2）が簡単ながらその誤りを指摘しているところをみると，海外でも表記の問題に関し必ずしも正しい理解がなされていないらしい。ともあれ眼科領域に広がつていると思われるかかる誤解を正すのも意義なくはないと考えた。

The Statement, that a correcting lens placed at the anterior focal point of an eye does not alter the size of the retinal image, is interpreted often falsely in Japan, as if it were true in the practice of spectacle correction : e. g. an ex-planation in which the focal point is replaced with 15 mm in front of the cornea, a false in-struction concerning the relation between the spectacle fitting and the eyelashes, etc. arc des-cribed.

The following three assumptions are neces-sary in order to keep the size of the retinal image constant by fixing the vetex distance.
