ガレーン動静脈瘤によると思われる視神経萎縮の1例

Bilateral Optic Atrophy due to Aneurysm of The Great Vein of Galen. A Casc Rcport

I．緒言

　脳血管の疾患はMillikanの分類によつても明らかなごとく種々のものがあるが，近年脳血管撮影の進歩により，比較的容易にその鑑別診断が可能となつてきた。その中で特に脳血管外科の対象となるものに脳動脈瘤，脳動静脈瘤，脳動静脈奇形（脳動静脈瘤）などがあげられるが，ガレーン動静脈瘤は脳動静脈奇形の中でも頻度が少ないといわれている。

　今回，著者らは両眼の視力障害を主訴として来院した患者について，椎骨動脈撮影の結果，ガレーン動静脈瘤を発見し，脳外科にてTolkildsen手術を施行したが，経過不良にて死の転帰をとつた1例につき，検討し，いささか知見を得たので，ここに報告する。

Progressive deterioration of vision has set in a 25-year-old male one month following a blunt head trauma inflicted in a traffic accident. Two years later, the patient was examined by the authors and far-advanced bilateral optic atro-phy was detected.

While bilateral carotid angiography showed normal vascular patterns, vertebral angiography led to the detection of an aneurysm in the great vein of Galen. The optic atrophy is ascribed to the cerebral arteriovenous aneurysm with the head injury serving as an accelerating factor.
