Sarcoidosisにおけるbetamethason使用時のリンパ節組織の変化観察の一例

Histopathological Study on the Lymphnodes Treated by Betamethason on a Case of Sarcoidosis

Ⅰ．緒言

　わが国におけるsarcoidosisの実態については，1960年にsarcoidosis臨時疫学調査班が第1回全国調査を実施してから，sarcoidosis研究協議会による第3回にわたる全国調査により，総数700例以上の多数のsarcoidosisが発見され，その後も増加する傾向にある^1）〜3）。これらの内，約半数になんらかの眼所見が報告され，欧米と比較して頻度が高い^4）5）。ゆえに本邦における，眼sarcoidosisの症例はもはや，まれではないと考えられる。多数の研究者がsarcoidosisの診断，病理，および治療について多くの報告をしているが，治療については，その多くがsteroid hormonを主体とし，効果判定は胸部X線所見あるいはリンパ節腫瘤の触診などによるものが多く，sarcoi—dosisの主体の病変であるリンパ組織の変化過程を，steroid hormonの投与と平行して観察したものはほとんど報告されていないようである。そこで私は，sarcoidosisの一症例をみる機会を得たのでsteroid hormon投与により，リンパ節の病変部にどのような推移がみられるかを観察したので報告する。

A 20-year-old male student sought medical. advice on account of bilateral visual disturbance of two months' duration. Ophthalmological exa-minations disclosed the presence of iridocyclitis. bilaterally, swelling of bilateral inguinal lymph nodes an dpulmonary type of sarcoidosis belong-ing to Group II a-b after Wurm-Heilmayer. Kveim test and tuberculin reaction were nega-tive. The inguinal lymph nodes were excised and histopathological findings revealed typical epitheloid cell nodules. The clinical diagnosis indicated sarcoidosis.
