絶対緑内障に対する減圧手術成績について

Results of Tension Lowering Operations for Absolute Glaucoma

I．緒言

　近年緑内障に対する診断的及び治療医学的分野に於ける進歩はめざましいものがある。特にトノグラフィー，隅角鏡検査及び種々の誘発試験等の発達によつて緑内障の予防医学的性格が強調され始めている現在に於ても，一般人の結核や成人病に対する知識に比較すると緑内障のそれははるかに劣つているのが実状である。事実私共眼科医が緑内障のため失明した患者に遭遇する機会は少なくない。就中，自覚的苦痛を有する絶対緑内障眼は眼科医の頭を痛める問題の一つである。

　絶対緑内障では眼球の緑内障性変化が高度となつているため，薬物治療，手術共に減圧効果が充分でないため，疼痛の激しいものに対してはとかく眼球摘出手術又は眼球内容除去術が考えられがちである。保存的手術を考えるとしても，絶対緑内障であるためその視機能に対する回復の望みは初めからないのであるから，手術の目的は減圧による自覚的苦痛の除去乃至軽減にあるのは勿論である。

Operative managements for tension-lowe-ring were done on thirty-one eyes with abso-lute glaucoma in these three years. The re-sults were as follows :

Out of twenty eyes treated by iridencleisis, the intra-ocular pressure was controlled and the subjective complaints subsided in seven-teen eyes (85.0%), without using any post-operative medical therapy. As to nine eyes treated by cyclodiathermy including two casesof angiodiathermy, five eyes (55.5%) proved successful. In cyclodialysis, so was one of two eyes.
