涙道形成術

An attempt to reconstruction the lacrimal drainage

Ⅰ．緒言

　鼻涙管閉塞の治療として，Chondron^1）　Hiru—doid^2）注入療法を報告したが薬物療法では回復しない症例がある。この様な重症例には涙嚢鼻腔吻合術が優れた治療法であることは既に認められている。しかし涙嚢鼻腔吻合術，涙嚢節骨洞吻合術の欠点は耳鼻科的器械，耳鼻科的知識が必要で，眼科医が単独で手術するには何か不安があることである。

　私は結膜に於ける皮膚弁の移植が長期間経過すると非常に良くなじむ点に着目し，涙道欠損部或は涙道閉塞部を拡張した粘膜損傷部にも皮膚弁を移植したらと考え実験し効果をあげたので報告する。

An attempt to reconstruct the lacrimal dra-inage channel by transplanting a skin flap from the upper arm is described. Satisfactory results were secured by rolling up the skin flap around the Nelaton's catheter (No. 3 to 6) and by inserting it into the dilated nasola-crimal duct in 3 cases of chronic dacryocy-stitis and one case of nasolacrimal occlusion.

In one case of absent lacrimal sac, a tube from the skin flap was sewn to the lacrimal canalicule in the one end with the other end reaching the nasal cavity. Functional im-provement in the lacrimal passage resulted.
