円錐角膜の臨床的研究

Clinical study of keratoconus

I．緒言

　円錐角膜は従来稀な疾患と考えられていたが単眼近視性乱視や矯正視力の出ない者等についてPlacido角膜計や細隙燈顕微鏡による検査や，或はコンタクトレンズ装用時のフローレスチンテスト，で見出される事があり決して稀な疾患ではない。

　本症は従来種々の観点から討論されいるが臨床的にはFranceschetti^1），　F．　Ridley^2）により報告され，遺伝学的には劣性遺伝病とされており，Amsler^3）はKeratophotographyによつて当疾患の病勢の進行時期を3種類に分類した。又教室の紺山^4）は200眼の円錐角膜につき故佐藤教授の分類^5）を治療的観点から詳細に補足修正し新らたな分類を行つた。

128 cases of conical cornea (consisting 119 cases of typical and 9 cases of atypical) is studied clinically and pathologically. In this study the typical 119 cases is devided intogroups according to corrected visual acuity (with contact lens), and slit lamp findings are recorded in each group : limbal blood ves-sel loops, epithelial covering change is stro-ma, nerve fibers, posterior surface, and Fleis-cher's ring. The results are compared in each group to establish the correct diagnosis in the earlier stage.
