（17）白内障摘出術後の隅角の所見について

Observations on the anterior chamber angle following cataract extraction.

　白内障手術に於ける切開創の状態を観察することは，創の治癒に至るまでの経過，その間に於ける併発症の発生の機構並にそれに対する処置或は予防法について考察する上に，重要な手がかりとなるのであるが，この切開創の位置の関係からして，外面の創の状態は視診により観察出来るのに反し，内面の創の状態は通常の斜照法や細隙燈顕微鏡検査法によつては観察出来ない。従つてこの内面の創の状態は，従来人眼ではたまたま摘出し得た眼球についての所見によるの他は，主として動物実驗の成績によつて推定して来たのであつて，この種の研究はWeinstein以来最近のDu-nnington等のものに至るまで多数あつて^{（1）（2）}，その結果創の治癒経過は合併症のない場合もある場合も略々明となつて居る。しかしこれらの成績を生体眼について観察することは更に有意義と考えられる。この内面の創は前房隅角の近くに存する関係上内面の創の状態及びその隅角部に及ぼす影響は，隅角視診法によれば生体眼に於ても観察出来るのであつて，Sugar^{（3）（4）}はこの方法の利用を推奨し，特に白内障手術後の無水晶体眼に起つた緑内障の眼の隅角について広汎な観察を行い，Kronfeld及びGrossmann^（5）も同様の観察を行つて居るが，一般に白内障手術眼の隅角についての観察は，私の検索し得た範囲では最近のOes-terle^（5）の報告以外には見当らないようである。

The anterior chamber angle is observed for the period of 2 to 4 weeks postoperatively following cataract extraction in 22 eyes and the results are summarized as follows :In cases that are uneventful either during or after the operation the recovery of the anterior chamber is normal excepting for a small ciliary or trabecular synechia that may be present at the superior portion of the chamber angle ; the scar of the wound of incision is seen only as a thin grayish line that lies anterior to the ring of Scwalbe. In such cases the chances are that the chamber angle becoming wider after the operation.
