Abstract. Purpose:To report glass fragments floating in the irrigating solution detected in a case during cataract surgery. Case:A 79-year-old male received uneventful phacoemulsification-aspiration in the left eye. During similar surgery in the right eye 2 weeks later, a foreign body was detected floating the anterior chamber. Findings:The foreign body was 1 mm×1.5 mm in size. The bottle of irrigating solution was found to contain 12 pieces of similar fragments ranging from 15 to 113μm in size. They were shown to contain silicone, oxygen and aluminium and were identified as glass material by energy-dispersive X-ray spectrometry. Conclusion:Due precaution is needed as glass fragments may contaminate irrigating solution for cataract surgery while cutting of the ampoule.
