要約　目的：角膜ジストロフィに対する治療的角膜切除術（PTK）の治療成績について検討する。症例：臨床的に顆粒状角膜ジストロフィと考えられ，角膜実質浅層にびまん性の混濁を有する症例に対しPTKを施行し6か月以上経過観察できた16例31眼で，手術時平均年齢67.3±10.6歳，術後平均観察期間26.0±25.0か月であった。結果：31眼中23眼（74％）に2段階以上の視力の改善を認めた。術後6か月の平均屈折値は術前と比べ＋0.72D遠視化し，術後比較が可能であった平均コントラスト感度はすべて改善した。結論：PTKは角膜実質浅層の病変に対して有効な治療法であると考えられた。

Abstract.　Purpose：We evaluated the treatment outcome of phototherapeutic keratectomy（PTK）for corneal dystrophy. Cases：This study included 31 eyes of 16 patients who was thought clinically granular corneal dystrophy and underwent PTK for corneal dystrophy presenting with diffuse opacity in the superficial corneal stroma with a minimum follow-up of 6 months. The mean age at surgery was 67.3±10.6 years. The mean follow-up period was 26.0±25.0 months. Findings：Best corrected visual acuity improved by 2 lines or more in 23 eyes of 31 eyes（74％）of cases. A hyperopic shift of ＋0.72D was observed 6 months after surgery. Contrast sensitivity in average improved postoperatively. Conclusion：These results suggest that PTK is an effective treatment for diseases in the superficial corneal stroma.
