網膜動脈閉塞症を発症し治療開始されたChurg-Strauss症候群の1例

A case of Churg-Strauss syndrome treated after manifestation of retinal artery occlusion

要約　背景：Churg-Strauss症候群では，気管支喘息や好酸球増加が先行し，全身性の血管炎が生じる。目的：治療開始前に網膜動脈閉塞症が発症したChurg-Strauss症候群の症例の報告。症例：50歳女性が急激な右眼の視力低下と視野障害で受診した。13年前から喘息，10年前に好酸球性肺炎があり，副腎皮質ステロイドを内服していたが，1年前から中断した。所見：矯正視力は右0.01，左1.5で，右眼の眼底後極部は白色化し，中心窩に桜実紅斑があり，蛍光眼底造影で網膜動脈閉塞症の所見があった。Churg-Strauss症候群と診断し，ステロイドのパルス療法を行った。9週後に白色網膜はなくなり，視力が0.1に回復した。以後2年後の現在まで，病状は安定している。結論：無治療のChurg-Strauss症候群に網膜動脈閉塞症が発症し，治療開始の契機になった。

Abstract.　Purpose：To report a case of Churg-Strauss syndrome with retinal artery occlusion（RAO）before initiation of treatment. Case：A 50-year-old female presented with abruptly impaired vision and visual field in her right eye. She had had bronchial asthma since 13 years before and eosinophilic pneumonia 10 years before. She had been receiving peroral corticosteroid until one year before. Findings：Corrected visual acuity was 0.01 right and 1.5 left. The right eye showed findings compatible with RAO. Pulsed steroid therapy was initiated under the diagnosis of Churg-Strauss syndrome. Opacity of the retina disappeared 9 weeks later with visual acuity improving to 0.1. She has been doing well for 2 years until present. Conclusion：This case illustrates that RAO may be pathogenetically involved with Churg-Strauss syndrome.
