迅速診断キットが補助診断として有用であった淋菌性結膜炎の1例

要約　目的：淋菌の迅速診断キットで早期に診断できた淋菌性結膜炎の症例の報告。症例：48歳男性が2日前からの右眼痛，眼脂，眼瞼腫脹で受診した。副鼻腔炎による眼窩蜂窩識炎と診断されていた。所見：両眼に多量のクリーム状の眼脂があり，右眼の眼瞼腫脹と発赤が顕著であった。眼脂の塗抹標本に，白血球に貪食されたグラム陰性双球菌がみられた。免疫クロマトグラフィ法を用いた迅速診断キット「クリアビュー・ゴノレア^®」が陽性を示した。淋菌性結膜炎を疑い，抗菌薬の点眼と内服を開始した。4日後に眼脂培養からNeisseria gonorrhoeaeが分離同定された。初診から8日後に軽快し退院した。結論：迅速診断キット「クリアビュー・ゴノレア^®」で淋菌性結膜炎の補助診断法として有用であった。

Abstract.　Purpose：To report a case of gonococcal keratoconjunctivitis who was diagnosed using a rapid diagnostic kit. Case：A 48-year-old male was referred to us for swollen eyelid,pain and discharge in the right eye since 2 days before. He had been diagnosed with orbital phlegmon the day before. Findings：Both eyes showed profuse creamy discharge. The right eye showed swelling and hyperemia. Smear from the discharge showed gram-negative diplococcic phagocytozed by leukocytes. A rapid diagnostic kit,Clearview Gonorrhoeae^®,showed positive for gonococcus,leading to the diagnosis of gonococcal conjunctivitis. Neisseria gonorrhoeae was cultured from the smear 4 days later. Topical and systemic treatment with antibiotics was followed by rapid improvement and cure 8 days later. Conclusion：Clearview Gonorrhoeae^® was useful in the rapid diagnosis of gonococcal keratoconjunctivitis.
