全身悪性リンパ腫の視神経乳頭浸潤に対してメトトレキサート硝子体腔注射が有効であった1例

Intravitreal injection of methotrexate was effective in a case of optic disc invasion of systemic malignant lymphoma

要約　目的：全身悪性リンパ腫の視神経乳頭浸潤にメトトレキサートの硝子体注射が奏効した1例の報告。症例：61歳女性が1週前からの右視力低下で受診した。22か月前に後頭蓋窩に原発した悪性リンパ腫に対し，腫瘍切除と化学療法を受けた。びまん性大細胞型B細胞性リンパ腫で，部分寛解中であった。所見：矯正視力は右0.4，左05で，右眼の視神経乳頭に発赤腫脹があった。同時期に悪性リンパ腫の頸髄転移が発見された。内科による4種類の抗がん剤治療は奏効せず，右眼視力はさらに低下した。初診から2か月後にメトトレキサートの硝子体注射を開始した。右眼視力は一旦改善したのち0.05に低下した。初診から5か月後にリンパ腫の脳幹への転移で不帰の転帰をとった。結論：全身悪性リンパ腫の視神経乳頭浸潤に対し，メトトレキサートの硝子体注射が奏効する可能性がある。

Abstract.　Purpose：To report a case of optic disc invasion of systemic malignant lymphoma that responded to intravitreal injections of methotrexate. Case：A 61-year-old female presented with impaired right vision since one week before. She had been diagnosed with malignant lymphoma originating in the posterior cerebral fossa 22 months before. It proved to be diffuse macrocellular B-cell lymphoma. She was improving after resection of tumor and systemic chemotherapy. Findings：Corrected visual acuity was 0.4 right and 0.5 left. The right optic disc showed hyperemia and swelling. Metastasis of lymphoma to the cervical spine was detected. After failure of systemic chemotherapy,she started receiving intravitreal injection of methotrexate. The right visual acuity showed temporary improvement. The patient deceased 5 months after her visit due to metastasis to the brain stem. Conclusion：This case illustrates that intravitreal methotrexate may be effective for optic disc invasion of malignant lymphoma.
