Abstract. Purpose:To report a case of optic disc invasion of systemic malignant lymphoma that responded to intravitreal injections of methotrexate. Case:A 61-year-old female presented with impaired right vision since one week before. She had been diagnosed with malignant lymphoma originating in the posterior cerebral fossa 22 months before. It proved to be diffuse macrocellular B-cell lymphoma. She was improving after resection of tumor and systemic chemotherapy. Findings:Corrected visual acuity was 0.4 right and 0.5 left. The right optic disc showed hyperemia and swelling. Metastasis of lymphoma to the cervical spine was detected. After failure of systemic chemotherapy,she started receiving intravitreal injection of methotrexate. The right visual acuity showed temporary improvement. The patient deceased 5 months after her visit due to metastasis to the brain stem. Conclusion:This case illustrates that intravitreal methotrexate may be effective for optic disc invasion of malignant lymphoma.
