Abstract. Purpose:To report changes in the morphology of macular hole after unsuccessful initial surgery as observed by optical coherence tomography(OCT). Cases and Method:Vitreous surgery has been performed on 62 eyes of 62 cases with idiopathic macular hole in the past 24 months. This retrospective study was made on 5 eyes with persistent macular hole after initial surgery. All the eyes received peeling of internal limiting membrane. The vitreous was replaced by air in 2 eyes and by sulfur hexafluoride in 3 eyes. Results:All the 5 eyes showed cyst and fluid cuff at the edge of macular hole before surgery. This finding persisted in 3 eyes and disappeared in 2 eyes after surgery. Conclusion:Cyst and fluid cuff may persist or disappear after unsuccessful surgery for idiopathic macular hole. There is a possibility that direction of traction may not be uniform in the pathogenesis of macular hole.
