Abstract. Purpose:To report 2 cases treated by one-port vitrectomy for lens-related complications. Case:A 69-year-old male had received cataract surgery followed by glaucoma treated by multiple surgeries. Penetrating keratoplasty resulted in iris prolapse and glaucoma. Another 67-year-old female presented with capsular block syndrome after cataract surgery and scleral buckling with macular plomb with a high risk of dislocation during another surgery. Clinical Course:Both eyes were treated by one-port vitrectomy by inserting the cutter through a valved 25 G trocar cannula connected to a 20 G infusion cannula. Results:Lesions posterior to the intraocular lens could be excised without intraoperative changes in intraocular pressure or other complications. Posteriorly dislocated lens cortex could be removed in the first case. Conclusion:One-port vitrectomy was safe and effective in removing lesions in anterior and posterior vitreous.
