要約　目的：3焦点眼内レンズ（AcrySof^® IQ PanOptix^® Trifocal）および連続焦点型多焦点眼内レンズ（TECNIS Synergy^®）の術後視力・屈折値変化を比較検討した。

対象と方法：対象は2019年2月〜2022年2月に，当院にてPanOptix^®（以下，P群）とTECNIS Synergy^®（以下，S群）を挿入した53例94眼のうち，白内障以外の眼科疾患および術中・術後トラブルがなく，術後3か月以上経過観察できた32例58眼とした。P群は28例50眼（72.2±8.0歳），S群は4例8眼（71.5±5.6歳）。方法は術後3か月の遠見（5m），中間（70cm），近見（30cm）裸眼logMAR視力，術後自覚屈折値（等価球面度数）を後ろ向きに調査し，術前予想屈折値（P群：−0.01±0.14D，S群：0.05±0.10D）と合わせて比較検討した。

結果：術後3か月の裸眼logMAR視力はP群遠見0.01±0.11，中間0.12±0.13，近見0.27±0.12，S群遠見0.03±0.05，中間0.12±0.16，近見0.29±0.21であった。遠見・中間・近見の裸眼視力は両群で有意差はなかった。術後3か月の自覚屈折値は，P群0.03±0.31D，S群−0.09±0.18Dであり，自覚屈折値−術前予想屈折値はP群で0.04±0.29D，S群で−0.14±0.21Dであった。

結論：P群，S群ともに遠見・中間・近見での良好な裸眼視力を得られた。また，P群は術前予想と術後3か月自覚屈折値の誤差は軽度であるが，S群では予想よりも術後3か月自覚屈折値は近視化する傾向であった。

Abstract　Purpose：We compared the clinical outcomes after cataract surgery with implantation two different interocular lenses, AcrySof^® IQ PanOptix^®Trifocal（Alcon）（here in after reffered to as PanOptix^®）and TECNIS Synergy^®（Johnson & Johnson）.

Methods：The medical records of patients undergoing cataract surgery with triforcal interocular lenses（PanOptix^®, Group P, n＝50 and TECNIS Synergy^®, Group S, n＝8）implantation between February 2019 and February 2022 and with the data available for the 3-month visit were reviewed without disease order than cataract and those who had problems during or after surgery. The outcomes of distance, intermediate, and near visual acuity（VA）, awareness refraction, and preoperation expectation refraction（Group P −0.01±0.14D, Group S 0.05±0.10D）were assessed at the 3 months follow-up.

Results：No statistically significant differences were found between distance logMAR VA（Group P 0.01±0.11, Group S 0.03±0.05；p＝0.65）, intermediate logMAR VA（Group P 0.12±0.13, Group S 0.12±0.16；p＝0.85）, near logMAR VA（Group P 0.27±0.12, Group S 0.29±0.21；p＝0.63）, awareness refrection（Group P 0.03±0.31D, Group S −0.09±0.18D；p＝0.29）, differences between awareness refraction and preoperation expectation refraction（Group P 0.04±0.29D, Group S −0.14±0.21D；p＝0.11）.

Conclusions：We obtained good distance, intermediate, near VA by naked eye in both the groups. The difference of the refraction value between the preoperation expectation and the operation in the Group P for 3 months was slight, but was a tendency to make the myopia than expectation in the Group S.
