Purpose:To report two cases of atypical type of idiopathic orbital inflammation. Cases and Findings:Both were females, were unilaterally affected, and were aged 70 and 58 years respectively. Initial visual acuity(VA)was no light perception(NLP)and 0.04 respectively. Computed tomography(CT)showed ill-defined tumor in the orbital apex. Pulse therapy with methylprednisolone was followed by improved VA from NLP to 0.3 in one case. Restricted ocular motility became normalized. After the treatment proved futile, the other case received systemic methotrexate. VA improved from 0.04 to 1.0 with normalized ocular motility. Tumorous mass in the orbital apex disappeared in both patients. Conclusion:Atypical type of idiopathic orbital inflammation is characterized by severe impairement of VA and ocular motility. It may respond to pulsed corticosteroid therapy or systemic methotrexate.
