Abstract. Purpose:To report a case who showed ophthalmoplegia followed by scleritis and hypertrophic pachymeningitis. Case:A 75-year-old female presented with headache, right eyelid ptosis and diplopia. Her right eye showed restricted inner and downward ocular motility, and scleritis. Posterior scleritis developed one month later. Visual auicty decreased to 0.1 due to optic perineuritis another month later. Gadolinium-assisted magnetic resonance imaging(MRI)showed hypertrophic pachymeningitis. Treatment with semipulsed prednisolone was followed by gradual remission of ptosis, diplopia and scleritis. Visual acuity improved to 0.8. Conclusion:Hypertrophic pachymeningitis is a possibility when scleritis is associated with ophthalmoplegia or headache. Gd-MRI was useful in the diagnosis in the present case.
