Abstract. Purpose:To report a case of internal carotid artery stenosis with ocular ischemic syndrome who manifested fluctuation of visual acuity following systemic hypotension after coronary bypass surgery. Case and Findings:A 72-year-old male presented with impaired visual acuity. He had had systemic hypertension, diabetes mellitus and angina pectoris. His corrected visual acuity was 0.7 right and 0.06 left. The left eye showed longstanding corneal opacity. He received coronary bypass surgery 2 months later. When awakening, the visual acuity was down to 0.1 right and 0.01 left. Both eyes showed rubeosis iridis. Intraocular pressure was within normal range. Further examinations showed stenosis of internal carotid artery, leading to the diagnosis of ocular ischemic syndrome. The right visual acuity fluctuated between 0.1 and hand motion when the blood pressure decreased below 100mmHg. Recovery of cardiac function with elevated blood pressure was followed by disappearance of rubeosis iridis in both eyes and recovery of right visual acuity to 0.2. Conclusion:Presence of stenosis of internal carotid artery may accelerate ocular ischemic syndrome. Careful systemic examinations are indicated when planning a major cardiac surgery.
