Abstract. Purpose:To report a case of sympathetic ophthalmia following vitrectomy for metastatic endophthalmitis. Case and Findings:A 59-year-old female presented with pain and blurring of visions in her right eye. She had had fever and painful swelling in her right foot since 3 days before. Her corrected visual acuity was hand motion right and 1.0 left. The right eye showed signs of endophthalmitis including hypopyon and vitreous opacity. She was diagnosed with metastatic endophthalmitis and received pars plana lensectomy and vitrectomy. Pneumococcus was cultured from the vitreous specimen. The right eye became phthisic 2 weeks later. She developed blurring and hyperemia in her left eye 3 months later. The left eye showed serous detachment. Cerebrospinal fluid showed pleocytosis. The left eye was diagnosed with sympathetic ophthalmia. Retinal detachment subsided 6 weeks after pulsed corticosteroid. Conclusion:Vitrectomy for metastatic bacterial endophthalmitis seemed to have induced sympathetic ophthalmia in the fellow eye.
