Abstract. Purpose:To report presumed cytomegalovirus(CMV)retinitis in a case of vasculitis associated with myeloperoxidase-antineutrophil cytoplasmic antibody(MPO-ANCA). Case:A 67-year-old female presented with impaired visual acuity in both eyes. She had been receiving corticosteroid and immunosuppressive agent for MPO-ANCA vasculitis with pulmonary alveolar hemorrhage and crescentic glomerulonephritis since 8 months before. Findings:Her corrected visual acuity was 0.6 right and 0.8 left. The left eye showed exudative lesions with hemorrhage in the inferior sector. Systemic and funduscopic findings were suggestive of CMV retinitis. Fundus lesions disappeared after 3 weeks of gancivlovir with improved visual acuity. There has been no recurrence until death due to intracranial abscess 6 months later. Conclusion:The present case is the first reported instance of CMV retinitis in an MPO-ANCA associated vasculitis. Systemic corticosteroid and cytostatic agent may have facilited CMV retinitis.
