ミエロペルオキシダーゼ抗好中球細胞質抗体関連血管炎に合併したサイトメガロウイルス網膜炎と考えられた1例

Presumed cytomegalovirus retinitis in a case of vasculitis associated with myeloperoxidase-antineutrophil cytoplasmic antibody（MPO-ANCA）

要約　目的：ミエロペルオキシダーゼ抗好中球細胞質抗体（MPO-ANCA）関連血管炎患者に併発したサイトメガロウイルス（CMV）網膜炎の報告。症例：67歳女性が両眼の視力低下で受診した。8か月前に肺胞出血と半月体形成腎炎を伴うMPO-ANCA関連血管炎と診断され，副腎皮質ステロイドと免疫抑制剤を内服中であった。所見：矯正視力は右0.6，左0.8であり，左眼眼底の下方に出血を伴う白色滲出性病変があった。左眼CMV網膜炎を疑い，ガンシクロビルの全身投与を開始した。左眼の滲出性病変は消退し，3週間後には右1.2，左1.0の視力に改善した。以後6か月後に脳内細菌性膿瘍で死亡するまで網膜炎の再発などはなかった。結論：本症例は，MPO-ANCA関連血管炎にCMV網膜炎が併発した初めての報告である。長期間の副腎皮質ステロイドと免疫抑制剤の全身投与がCMV網膜炎の原因になった可能性がある。

Abstract.　Purpose：To report presumed cytomegalovirus（CMV）retinitis in a case of vasculitis associated with myeloperoxidase-antineutrophil cytoplasmic antibody（MPO-ANCA）. Case：A 67-year-old female presented with impaired visual acuity in both eyes. She had been receiving corticosteroid and immunosuppressive agent for MPO-ANCA vasculitis with pulmonary alveolar hemorrhage and crescentic glomerulonephritis since 8 months before. Findings：Her corrected visual acuity was 0.6 right and 0.8 left. The left eye showed exudative lesions with hemorrhage in the inferior sector. Systemic and funduscopic findings were suggestive of CMV retinitis. Fundus lesions disappeared after 3 weeks of gancivlovir with improved visual acuity. There has been no recurrence until death due to intracranial abscess 6 months later. Conclusion：The present case is the first reported instance of CMV retinitis in an MPO-ANCA associated vasculitis. Systemic corticosteroid and cytostatic agent may have facilited CMV retinitis.
