レーザーポインターによる心因性視力障害の1例

A case of psychogenic visual impairment induced by a laser pointer

目的：レーザーポインターでレーザーを照射された後に視力障害が生じた症例の報告。症例：12歳女児が同級生に右方向からレーザーポインターでレーザーをごく短時間照射された。その直後に右眼視力低下と視野欠損を自覚した。その10日後に当科を受診した。所見：矯正視力は両眼とも1.2であり，動的視野検査で異常はなく，眼底検査，OCT，蛍光眼底造影，多局所網膜電図，頭部MRIでも異常はなかった。その後も右眼の裸眼視力低下と再現性がない視野異常を訴えたが，他覚的に異常所見はなく，レンズ打消し法で良好な視力が得られた。結論：レーザー照射が誘因となった心因性視力障害と推定される症例である。

Purpose：To report a case of presumed psychogenic visual impairment following irradiation by a laser pointer. Case：A 12-year-old girl received a very short irradiation by a laser pointer. She noted visual disturbance and visual field defect in her right eye. Findings：When seen 10 days later,her corrected visual acuity was 1.2 in either eye. No abnormal findings were detected by funduscopy,optical coherence tomography,fluorescein angiography,multifocal electroretinogram,magnetic resonance imaging of brain,and dynamic perimetry. She continued to complain of impaired unaided visual acuity in her right eye and non-reproducible visual field defect. She had full corrected visual acuity throughout. Conclusion：Casual exposure to laser may have induced psychogenic visual impairment in this patient.
